Pregnancy Adjusted D Dimer

Pregnancy Adjusted D Dimer

3/19/2018  · “…D-dimer should not be recommended for use in the diagnostic work-up of PE in pregnancy.” Clinical Take Home Point: Among pregnant women presenting with concern for pulmonary embolism in pregnancy, there remain no data supporting the use of D-dimer in any diagnostic algorithm. D-dimer is a biomarker used as an exclusion criterion of VTE disease, but its usefulness during pregnancy shows limitations because D-dimer levels physiologically increase through pregnancy . The aim of our study was to follow the changes of D-dimer levels and to establish trimester-specific reference intervals during normal pregnancy .

8/12/2019  · The proposed strategy was to perform D-dimer testing on pregnant women with a pretest probability less than 40% (Wells score less than or equal to 4), and a negative PERC rule with an adjusted heart rate cutoff of 105 bpm. Adjusted D-dimer thresholds of 750/1000/1250 ng/ml for first, second and third trimester respectively were recommended.

Some causes of an elevated D-dimer include deep vein thrombosis (DVT), pulmonary embolism (PE), disseminated intravascular coagulation (DIC), cancer, underlying inflammation, and cardiac, renal or liver disease . Normal pregnancy is associated with alterations of the hemostatic system toward a hypercoagulable state. Elevated markers of coagulation and fibrinolytic system activation, such as D – dimer , indicate increased thrombin activity and increased fibrinolysis following fibrin formation throughout pregnancy .

This decreasing specificity can be explained by the physiological rise in the d -dimer level that commonly occurs during pregnancy. 7 At the time of presentation, a …
